Assertive Outreach
– Blue Mountains – Nepean – Hawkesbury –
Platform’s Assertive Outreach Team provides street-based services to young people between 16-25 years of age in the Nepean, Blue Mountains, and Hawkesbury LGA’s who are sleeping rough or couch surfing.
The Assertive outreach model uses deliberate targeting of people who are experiencing homelessness or at risk of homelessness and who are otherwise disengaged from community-based services, and works to identify the locations of vulnerable people, assessing their needs, and assisting them overcome barriers which may prevent them from resolving their homelessness issues and moving forward with their lives.
