Transitional Housing

– Blue Mountains – Hawkesbury –

Platform in partnership with Link Wentworth Housing (LWH), supports young people aged 17- 24 years old living in the Blue Mountains and Hawkesbury, who are homeless or at risk of homelessness and or experiencing housing stress. The program provides the opportunity to lease a property through LWH with case management support (provided by Platform) to maintain this tenancy.

The program offers those that are eligible up to 12-month in a tenancy with the aim to move into the private rental market or other alternative housing at the end of the LWH tenancy. Case Workers provide case management and accommodation assistance, information & advice, assessment & supported referral as well and support with skills development, maintaining education &/or employment assistance to maintain the tenancy

– Penrith –

In partnership with Mission Australia (MA) -The Grove Youth Outreach and Support Service. The Grove supports young people aged 16-25 years in the Penrith/Nepean, who are homeless or at risk of homelessness and or experiencing housing stress. Providing Case management and accommodation assistance, information & advice, assessment & supported referral & skills development. In this partnership young people can access a mix of transitional and longer-term housing options via Platform, MA Housing and Link Wentworth Housing.
